Has anyone here ever experienced a vasectomy failure?

I don't mean not waiting long enough or messing around on the side. I am asking if anyone has personally gotten pregnant after your hubby has had a vasectomy. I hear stories, but never first hand, and I am suspecting that they are more urban legend than truth.
