How many forms of varicella are there?

I have a germ project to do for my health cl and this is one of my question so i was wondering how many forms there exactly are if anyone knows. I have tried searching and searching and nothing comes up except chicken pox and shingles but nothing about how many forms there are of varicella. One more thing we have to dress up as this germ from head to toe full on and i was wondering if anyone had any ideas. I am not a very creative person so anything will help. Thanks in advanced.
